Add back "include direct mail campaign" on request form for Survey Options

Reached out to Angie on this. I think that when we removed this in and also added this checkbox to Seminar, the information was not changed to be sent to the PDF correctly.

Moving this to Needs Addl Information.

I’d like to set a short meeting to go over this in person with Angie and see what is fully expected.

Test example also automatically displays ‘No’ on the PDF that the Direct Mail Campaign is not included.

Spoke with Angie on this issue, we’re not quite sure if this checkbox was actually on this screen for the Survey option or if all items were always marked as “Yes” for Direct Mail.

I’m going to get with a developer and see if we can tell in the code if that checkbox was there previously or if there’s just a disconnect.

Just to clarify, you expect that the Include Direct Mail Campaign checkbox display on the following Survey Options:

• Letter Survey with Lunch and Learn Insert

• Standard Survey with Advance Planning Seminar Insert Card

• Standard with Lunch and Learn Insert

• Survey with Seminar Insert

• Other

Along with the following other options that currently have this checkbox:

• Lunch and Learn

• Seminar
